Undergraduate Music Major and Minor Admissions

Submit School of Music Audition Application (available in pdf format)
Submit School of Music Recommendation Form (available in pdf format)
Transfer students currently enrolled in a NASM Accredited college or university, you must complete a Transfer Acknowledgement Form (available in pdf format)
Repertoire Recommendations (available in pdf format)

Graduate Admissions

All candidates for graduate degrees in music must first submit an Application for Admission to the Graduate School at Illinois State. The application form must be submitted online or downloaded and mailed to the Admissions Office at the address on the form. In addition, graduate students must also submit a School of Music Graduate Application. This application may be downloaded and mailed to the School of Music at the address on the application.
